Corrections and right of reply

If we have published something about your facility that is wrong, out of date, or missing context, we want to know and we will fix it. That applies whether you are an operator, an administrator, a regulator, a resident or a family member.

How to reach us

Call or text (888) 538-0158 and say you are calling about something published on this site. Calls are answered 8am–8pm, seven days a week. Texts are answered at any hour, day or night. Ask for the watchdog team rather than the intake team — they are not the same people, and the watchdog team is who handles this.

What happens next

  1. We acknowledge that we have received it.
  2. We go back to the underlying record — the CMS file, the state licensing data, or whatever the original source was — and check the specific point you have raised.
  3. If we got it wrong, we correct the page and say so on it, with the date. We do not silently edit.
  4. If the record itself has changed — a finding overturned on appeal, a rating updated, an ownership change — we update to match and note the change.
  5. If we think the page is right, we tell you why, and we will publish your response alongside it if you want us to.

A note for facility operators

Most of what we publish about a specific home comes straight from the federal inspection record and the government’s own star ratings. If a finding is under appeal, or has been overturned since the file we downloaded, that is exactly the kind of thing we cannot see from the data and you can tell us in a sentence. We would rather hear from you than be wrong.

We date-stamp every figure for this reason. If the date on a page is older than the change you are telling us about, that is usually the whole explanation, and the fix is quick.
